Community Health Needs Assessment

At Dignity Health, a key part of our mission is supporting the health of our communities in meaningful, measurable ways. It is also part of our mandate as a non-profit healthcare provider under the Patient Protection and Affordable Care Act. That is why each and every one of our hospitals goes to great lengths to study and ascertain the most pressing health needs of the communities they serve. 

Dignity Health East Valley Rehabilitation Hospital had the opportunity to collaborate with St. Joseph’s Hospital and Medical Center’s (SJHMC) Community Benefit team to lead a community health needs assessment for Maricopa County, Arizona.

This process and its conclusions are described in a document called the Community Health Needs Assessment (CHNA), which is updated every three years.  According to the Centers for Medicare and Medicaid Services (CMS), CHNA refers to a state, tribal, local or territorial health assessment that identifies key health needs and issues through systematic, comprehensive data collection and analysis. The ultimate goal of this assessment is to develop strategies to address the community’s health needs and identified issues.
